The Soviets had a problem in the 1980s. Their cosmonauts couldn't be sedated. Their spec ops couldn't be amnestic. Their pilots couldn't develop tolerance over a six-month deployment. And the entire benzodiazepine class did all three of those things on a Tuesday.

So the Institute of Molecular Genetics in Moscow, working with the Zakusov Research Institute of Pharmacology, took a different approach. They started with tuftsin, a four-amino-acid fragment your own immune system cleaves off the heavy chain of immunoglobulin G. Tuftsin already had immunomodulatory bona fides. The team, led by Nikolai Myasoedov, bolted three more amino acids onto the C-terminus (Pro-Gly-Pro) to keep enzymes from chewing it apart.

The result was a heptapeptide called Selank:

Thr-Lys-Pro-Arg-Pro-Gly-Pro

What they expected was a more stable immune peptide. What they got was an anxiolytic that, in human trials, performed comparably to a benzodiazepine without sedation, without amnesia, without tolerance, and without the withdrawal syndrome that sends a meaningful fraction of long-term benzo users to the ER when they try to quit.

That was 1989-ish. Selank has been registered in the Russian Federation as Selanc for generalized anxiety disorder and neurasthenia for decades. American psychiatry, which writes tens of millions of benzodiazepine scripts a year, has not run a single phase 3 trial.

That gap is the problem.

The actual evidence

Russian peptide literature gets dismissed in the West as a category. That's lazy. Some of it is thin. Some of it is rigorous. The Selank human data is rigorous enough to deserve a hearing.

Zozulia et al., 2008 — GAD and neurasthenia trial

Sixty-two patients with GAD or neurasthenia. Active-comparator design (not placebo, which matters: medazepam is a real benzodiazepine, not sugar water). Thirty patients on Selank, thirty-two on medazepam. Hamilton Anxiety, Zung, and CGI scales. Anxiolytic effect was similar between the two arms. Selank also produced antiasthenic and psychostimulant effects that medazepam did not. Patients with low baseline leu-enkephalin half-life, the biomarker that correlated with severity, showed the strongest response on Selank.

Medvedev et al., 2015 — Phobic anxiety and somatoform disorders

Compared Selank combined with phenazepam against phenazepam monotherapy in patients with phobic anxiety and somatoform disorders. The combination arm showed superior anxiolytic effects, and those effects persisted one week after the last dose. Quality-of-life scores improved. Phenazepam, the comparator, is a Russian benzodiazepine roughly five times more potent than diazepam. Selank improved outcomes on top of it.

Volkova / Slominsky et al., 2016 — GABAergic gene expression

Frontiers in Pharmacology. This one is in English and indexed properly. Gene expression analysis showed Selank modulates expression of genes in the GABAergic system within one hour of administration. Critically, the pattern looked like allosteric modulation, not direct agonism. That distinction is why Selank doesn't produce the receptor downregulation and rebound hyperexcitability that benzos produce after chronic use.

Mechanism stack

GABA-A receptor allosteric modulation — enhances inhibitory neurotransmission without direct receptor binding, avoiding the sedation-amnesia-dependence triad
Enkephalinase inhibition — raises endogenous opioid tone without exogenous opioids
BDNF upregulation in hippocampus — promotes neuroplasticity and long-term anxiolytic remodeling
IL-6 and Th1/Th2 cytokine balance shifts — the immunology heritage from tuftsin
Serotonin metabolism induction — influences tryptophan hydroxylase expression without SSRI-style receptor flooding

That's not one drug's worth of activity. It's a network intervention. Which is exactly what you'd expect from a regulatory peptide rather than a small-molecule receptor lock-and-key.

Why this matters in 2026

The FDA put a boxed warning on the entire benzodiazepine class in September 2020. Black box. Dependence. Withdrawal. Death from misuse. The warning didn't reduce prescribing in any meaningful way, because clinicians don't have a good alternative for the patient sitting in front of them at 4 PM on a Thursday with panic disorder who needs something that works tonight.

SSRIs take six weeks to work and a third of patients can't tolerate the sexual side effects, weight gain, or emotional blunting. Buspirone is fine for mild GAD and useless for panic. Hydroxyzine helps some people sleep. Beta blockers handle situational performance anxiety and nothing else.

So clinicians keep writing Klonopin. Patients keep escalating. And the cycle continues because the alternative class, the regulatory peptides, sits in a regulatory gray zone that American clinical research has refused to enter for forty years.

The question is not whether Selank works. The Russian data answers that. The question is whether American clinicians are going to replicate the work on American patients with American methodology and American transparency, or whether we're going to keep ceding the entire peptide category to gray-market actors and a regulatory body that, understandably, doesn't trust influencer-grade evidence.

The Category 2 problem

In 2023 the FDA placed Selank acetate on the Category 2 list of bulk drug substances under 503A. Category 2 means significant safety concerns prevent inclusion on the approved compounding list. The stated concerns were immunogenicity (it's a peptide derived from an immune fragment) and insufficient data.

Both concerns are reasonable. Both are also solvable.

Immunogenicity in regulatory peptides is an empirical question. You measure it. The Russian post-marketing data from twenty-plus years of clinical use does not describe a clinically meaningful immunogenicity signal. That doesn't mean the FDA is wrong to want to see the work done again under their methodology. It means the work needs to be done.

Insufficient data is the same story. The Russian trials are real but they are small, single-country, and published in journals American psychiatrists don't read. The 2008 Zozulia trial would not pass FDA scrutiny as a registration trial. It wasn't designed to. It was designed to support Russian Ministry of Health registration, which it did.

The path forward is not to argue with the FDA about the existing data. The path forward is to generate new data. IRB-supervised. Prospective. American patients. Standardized outcome measures. All findings published, including the boring ones and the negative ones.

What we're doing

That's the boring version of disruption. It's also the only version that ends with the FDA having a reason to revisit the Category 2 placement.

The peptide space is full of people selling certainty they haven't earned. We're not in that business. Selank is a compound with strong preliminary human evidence, a forty-year safety record in another jurisdiction, and a mechanism that actually maps to the unmet need. It deserves rigorous American research. It is not yet ready for casual clinical use outside a research framework, and anyone telling you otherwise is selling something.

Replicate the evidence. Or watch the category get restricted.

The benzo problem is real. Selank is one of maybe three candidate solutions sitting on the shelf with enough preliminary evidence to justify the work. The work is what's missing. Not the molecule.

What patients should know

If you're on long-term benzodiazepines and looking for a way out, do not stop them on your own. Withdrawal can kill you. Find a physician who taper-manages benzos for a living and start there.

If you're considering Selank as part of a research protocol, ask the clinician three questions:

1. What IRB are you under?
2. What outcome measures are you tracking?
3. Where will the data be published?

If they can't answer all three, you're not in research. You're in retail with extra steps.

The compound is real. The mechanism is interesting. The Russian trials exist and are worth reading. The regulatory situation in the US is real and not going to resolve through outrage tweets. The work has to be done.

That's what responsible disruption looks like.
